What should I do when I wake at 3 a.m. and can't get back to sleep?

Do not check the time, do not solve anything, and do not lie there fighting. Belly breathing with a long exhale for ten minutes, counting exhales to ten. If you are still awake and wound up after twenty, get up, go somewhere dim, do something dull on paper, and return when sleepy. The 3 a.m. mind is a bad advisor, and the room is built on that fact.

The steps

  1. Do not look at the clock or the phone. The number makes it worse.
  2. Roll onto your back or side, hand on the belly.
  3. Exhale long through the nose, inhale small. Count exhales to ten. Start over.
  4. If a problem arrives, say the slate sentence: it is 3 a.m., this is not when I solve things.
  5. Twenty minutes and still wired: get up, dim light, boring task, no screens. Back to bed when sleepy.
  6. In the morning: do not pay for the night by sleeping in. Same wake time.

When not to, and when to get help instead

Waking at 3 a.m. with your heart pounding, night after night, can be anxiety, alcohol, apnea, menopause or a medication; it is worth a doctor if it is new or persistent. If 3 a.m. is when the dark thoughts come and they are about not wanting to be here, call or text 988 in the U.S., tonight, not tomorrow.

What the research says

The get-up-after-twenty-minutes rule is stimulus control from CBT-I, one of its best-supported pieces. Slow breathing raises vagal activity and improved sleep measures in a small insomnia study. The no-clock rule is older than any of it and every sleep clinician repeats it.

Other methods people use

CBT-I, delivered by a clinician or a structured app, is the treatment. Keeping a notepad by the bed to park the 3 a.m. idea. Earlier alcohol cut-off, since alcohol causes exactly this wake.

Why do I always wake up at 3 a.m.?

Sleep gets lighter in the second half of the night, and anything that nudges you (alcohol wearing off, a hot flash, a worry, apnea) lands then. Persistent 3 a.m. waking is worth a doctor’s look.

Should I get out of bed if I can't sleep?

After about twenty minutes of being awake and wound up, yes. Dim light, dull task, back when sleepy. It feels wrong and it works.
