What breathing slows a racing or pounding heart?

A long, slow exhale. The heart slows on every out-breath through the vagus nerve, and lengthening the exhale lengthens the slow part. Four in, eight out, through the nose, for two minutes. If you can feel the pulse in your neck, put two fingers there and watch it fall across the exhales; that is the feedback loop the book calls the brake.

The steps

  1. Sit. Lean back. Feet flat.
  2. Inhale small through the nose for about four.
  3. Exhale through the nose or pursed lips for about eight. Do not force the last of it out.
  4. Two minutes. Count the exhales if the mind needs a job.
  5. Optional: fingers on the neck pulse. Feel it slow on each out-breath.
  6. Then a Soundcheck: what set it off, and where are you still holding.

When not to, and when to get help instead

A pounding heart with chest pain, fainting, pain down the arm, or shortness of breath at rest is an emergency, not a breathing exercise. A heart that races at random, without a trigger, repeatedly, is a doctor’s visit for an ECG; some arrhythmias feel exactly like anxiety. Not while driving.

What the research says

Respiratory sinus arrhythmia, the heart slowing on exhale, is almost entirely vagal, shown under drug blockade in six volunteers. A longer exhale increased relaxation and vagal measures in two lab studies. Slow breathing lowered heart rate in the pooled trials. The physiology here is about as settled as anything on this site.

Other methods people use

The Valsalva maneuver and cold water on the face are what clinicians use for certain fast rhythms; they are not for self-diagnosis. Box breathing works if you are sitting and waiting. Caffeine and nicotine are the commonest reasons a healthy heart pounds.

Why does exhaling slow your heart rate?

The vagus nerve applies its brake on the out-breath and eases on the in-breath, so a longer exhale means more time on the brake. It is called respiratory sinus arrhythmia and it is normal.

How do I tell anxiety heart racing from a heart problem?

You often cannot, which is why a first episode or any episode with chest pain, fainting or breathlessness at rest gets a doctor. Once a doctor has cleared you, the exhale is the tool.
