How do I calm down fast when anxiety spikes?

Two inhales through the nose, the second a small top-up, then one long exhale through the mouth, as if fogging a mirror. That is the physiological sigh and it works in about four seconds. Do it once or twice, then let the next few breaths go out longer than they come in.

The steps

  1. Inhale through the nose until the lungs feel full.
  2. Sneak in a second, smaller inhale on top.
  3. Exhale through the mouth, slowly, all the way out. Let it take twice as long as the inhales.
  4. Repeat once. Then breathe normally with the out-breath slightly longer than the in-breath for a minute.
  5. If you can, drop your shoulders and unclench your jaw on the next exhale.

When not to, and when to get help instead

Not while driving; pull over first. If the double inhale feels uncomfortable, which it sometimes does during panic, use a single slow inhale and a long exhale instead. If spikes like this are frequent or come with chest pain, see a doctor; the breath is a tool, not a diagnosis.

What the research says

The body produces a sigh on its own every few minutes to re-inflate the lungs, and doing it on purpose drops arousal quickly. A 2023 Stanford trial found five minutes a day of it improved mood over a month more than mindfulness did, and a 2025 pilot found four minutes of it eased pain in a clinic waiting room. The long exhale underneath it is where the vagus nerve applies the brake; two small studies found a longer exhale changes how relaxed people feel independent of pace.

Other methods people use

Box breathing (four in, hold, four out, hold) steadies people who have to sit still, and the holds are skipped by anyone with panic. 4-7-8 is a long exhale with a long hold, mostly used for sleep. Coherent breathing at six a minute is the same pace I teach, with a bigger trial literature behind it. Alternate-nostril breathing works for people who like a hand task. All of them share the slow exhale; pick the one you will actually do.

How fast does the physiological sigh work?

Most people feel the first drop within one or two sighs, inside ten seconds. If it does not move the needle, do a Soundcheck and find where you are bracing; the breath cannot reach a jaw you are holding shut.

Why do I feel worse when I try to take a deep breath?

Because a big inhale is the wrong direction. Anxiety is already an inhale posture. Make the exhale the long part and the inhale small.
